Clergy Withholdings Worksheet: It's important for pastors and church treasurers to be aware of each active plan participant's health plan enrollments and necessary payroll witholdings. To that end, feel free to use this worksheet to calculate monthly amounts to be withheld from their pay for Health, Dental, Vision insurances as well as Health Savings Account (HSA) contributions. The Conference Benefits Office does not need a copy of this form – it is solely for your planning purposes. (Note: This form does not enroll participants in any plans.)

Health Benefits & Pension News
New DPP Group: A new Diabetes Prevention Program group is forming and will begin in January 2019. Join for 16 weekly/bi-weekly virtual meetings and then 6 virtual monthly follow-up meetings with a Trained Certified Diabetes Lifestyle Coach. During the program, ongoing coach support will help you to maintain healthy lifestyle changes. For more information, see this flier: Balanced Wellness DPP
New HSA Bank Information: In order to be a part of the bulk transfer, the deadline to submit your HSA authorization form (HSA transfer form) is January 15, 2019. You will need your new ActWise bank account number with PNC before you can submit the form. Please look to recieve this information from Anthem in mid-December. Once you have received your information please return the form to the Conference office (and not the address on the form). UMPIP Contributions: Reminder to all UMPIP participant who also receive CRSP benefits, make sure you are contributing your 1% to get your CRSP match. Also, for clergy who changed appointments this year and/or incurred moving expense, those moving expenses are "pensionable" income. Thus, be sure to contribute an additional 1% of those moving expense in order to ensure full CRSP match benefits.

Frank King Fund: On Saturday, December 1, 2018, the Frank King Fund grant window opens. The application and guidelines are posted on our website at www.kyumc.org/kinggrant.
Frank C. King was a Pastor of the former Kentucky Conference. In his will he left a trust to provide assistance to churches of the Kentucky Conference for additions and improvements for church facilities. The Conference Trustees have established a grant process to help facilitate the wishes of Rev. King. The grant is a matching grant which will provide up to $3,000 to selected recipients.
